How to create forms and share them with a Client

Therasee allows you to create reusable forms that can be shared with clients to collect information and track progress. Some example forms you can create include:

  • Client agreements

  • Intake forms

  • Assessment forms

  • Consent forms

  • Progress notes

  • Treatment plans

  • Client feedback forms

  • Risk assessment forms

  • Session feedback forms

  • Referral forms

Creating a Form Template

You can create form templates to reuse with different clients, saving time and ensuring consistency.

Practice Templates

If you are part of a team, you can create practice-wide templates that all users can access. Solo practitioners can skip this step and create templates in the "My Templates" section.

  1. Create a Practice Template:

    • Go to "Forms" from the sidebar and select "Practice templates" from the menu.

    • Click the "New template" button.

My Templates

You can add practice templates to your personal templates and make changes to them. You can also create new personal templates directly.

  1. Add Practice Templates to Personal Templates:

    • Go to "Practice templates" and click "Add to my templates" next to the desired template.

  2. Create a Personal Template:

    • Go to "Forms" from the sidebar and select "My templates" from the menu.

    • Click the "New template" button.

Creating a Form

Therasee's form builder includes all the elements you need to create comprehensive forms.

Form Builder Overview

  1. Top Menu:

    • Edit the form name by clicking the edit icon.

    • Click "Preview" to see what the form will look like for clients.

    • Click "Save" to save your changes.

    • Click "Publish" to make the form available to all team members (only for practice templates).

  2. Elements Side Panel:

    • Add elements by clicking or dragging them into the form. Elements include:

      • Page Elements: Text elements like headers, paragraphs, and dividers.

      • Question Elements: Input elements like short answers, long answers, dates, and multiple-choice questions.

  3. Preview:

    • Click "Preview" to see how the form will appear to clients.

  4. Select Category:

    • Organize forms by selecting a category from the dropdown in the menu bar.

  5. Save:

    • Remember to save your form by clicking the "Save" button before closing.

Creating a Form for a Client

  1. Navigate to Client Profile:

    • Go to the client profile you want to add the form to.

    • Click the "Forms" tab and then "Add form."

  2. Select or Create a Form:

    • Choose to create a blank form or use a template from "My templates" or "Practice templates."

    • Click "Create blank form" or "Use with client" next to the desired template.

  3. Editing and Sharing:

    • The form will appear in the client's list of forms. Click the more options icon next to the form and select "Edit" to make changes.

    • To fill out or sign the form, select "Fill and sign" from the dropdown.

    • Click "Share" to send the form to the client. The client will receive a notification with a link to complete the form.

  4. Notifications:

    • Ensure "Client submits a form" is switched on under Notifications in "My settings" to receive alerts when a client completes a form.

Viewing and Downloading Forms

  1. Viewing a Form:

    • Click on the form name in the forms list to view it.

  2. Downloading a Form:

    • Click "Download" from the more options dropdown or the "Download" button in the preview to save the form.

Client's Experience with the Form

Receiving the Form

  • Notification: The client will receive a notification to complete the form via the method you selected.

  • Accessing the Form: Clients can fill in the input fields and sign the form if needed.

Submitting the Form

  • Form Submission: Once the client submits the form, you will receive a notification.

  • Viewing Responses: The practitioner can see the completed form through their portal.

Downloading the Completed Form

  • Client and Practitioner Access: Both the client and practitioner can download the finished form.

By following these steps, you can efficiently create, manage, and share forms with your clients in Therasee, ensuring smooth information collection and progress tracking.
